Case Study: เมื่ออาการปัสสาวะขัด นำไปสู่ข้อเข่าอักเสบ
ล่าสุดมีคนไข้วัยกลางคนมาปรึกษาผมด้วยอาการ “เข่าซ้ายบวมเป่ง” ตรวจร่างกายพบว่าข้อเข่ามีความร้อนชัดเจน แต่ที่น่าสังเกตคือคนไข้มีประวัติ ปัสสาวะแสบขัด นำมาก่อนหน้านี้

การส่งตรวจเพาะเชื้อ: แม้ต้องรอผลแล็บประมาณ 1 วัน แต่ในทางปฏิบัติ หมอจะเริ่มให้ ยาฆ่าเชื้อ (Antibiotics) ทันทีร่วมกับยาลดอักเสบ เพื่อไม่ให้ทำลายกระดูกอ่อนในข้อเข่า

Beware! Acute knee swelling and inflammation may not just be a “joint” issue, but a warning sign of an infectious disease.
Have you ever experienced a sudden, swollen, red, hot, and painful knee that makes walking almost impossible? Many people might only think of gout or osteoarthritis, but as an internal medicine doctor, I want to share an interesting case that’s more common than you might think.
Case Study: When painful urination leads to knee inflammation
Recently, a middle-aged patient consulted me with a severely swollen left knee. Physical examination revealed significant warmth in the knee joint. Interestingly, the patient had a history of painful urination prior to the diagnosis.
Based on internal medicine diagnosis, I suspected septic arthritis, or infectious joint inflammation, especially from active sexually transmitted infections (such as gonorrhea), which can spread through the bloodstream to the knee joint.
Treatment procedure: Drainage to reduce pain and for diagnosis.
Arthrocentesis: I performed an aspiration of synovial fluid from the knee. It was found to be “cloudy,” clearly indicating infection.
Culture testing: Although lab results take about a day, in practice… The doctor will immediately start antibiotics along with anti-inflammatory medication to prevent damage to the cartilage in the knee joint.
Post-treatment results: Immediately after draining the excess fluid, the patient feels much better, the pain decreases, and the knee swelling is noticeably reduced.
